I,  ,  the model whose personal information appears above, am fully aware that this information I supply may be used by OzGirlz Marketing and Promotions to promote myself and the Agency to potential clients. I understand that a portion of this information and images that either I supply or those taken by the agency, their photographers, or clients will appear on the Agency website ( www.ozgirlz.com.au) and maybe contained in any brochures promoting the Agency and its services. I understand a Profile will be created on my behalf for me, using this information and any photos and this will appear under my name in the model gallery. I authorise the use of this information and images in the manner it is intended.Â

SECURITY WARNING:Â Â Â Although OzGirlz Management makes every attempt to ensure all information received by our office is correct (even by recording the details of the computer that submits this information in case of legal purposes later), we still require further information that only you should know.

Please note: Ozgirlz Management will never under any circumstances allow any model to be forced or pressured into any Job or Photograph by any staff member, client or photographer of the agency. Should the model feel as though this is happening or feels uncomfortable at any request, they are to cease the activity and immediately seek advice and consult OzGirlz senior management. The agreements held in good faith between OzGirlz Management and the model strictly prohibit such actions toward a model and the model always carries the right to refuse any or all work or requests. All authorisations for photographic images and work carried out by the model are done so by the model under their own decision processes
